How Do Families Submit Necessary 4K Forms?

Because our district is not holding open house events at the 4K locations, there are several ways you can submit the forms (proof of residency-driver's license or utility bill; immunization record; and verification of age-birth certificate) electronically or in the mail.

  1. Scan the forms and email to the elementary school that your 4K student will attend
  2. Take a picture with your phone and email those photos to the elementary school 
  3. Make paper copies of the forms and send them in the mail to the elementary school
